About Form A

Use Form A if you wish to apply for a passport for your child, if your child was born on the island of Ireland (including Northern Ireland) on or after 1 January 2005.

By completing this form you declare that you were legally resident on the island of Ireland (including Northern Ireland) for 3-out-of-4 years immediately before the birth of your child.

To make the declaration, download and complete Form A.

Passport application

To apply for your child's passport, send the Declaration of Residence and passport application form to the Department of Foreign Affairs and Trade for processing. (Do not send to INIS Citizenship Division.)
